Entry Form

Below is the form that needs to be submitted to be entered into the the tournament.  You will not be entered into the tournament unless you submit this form.  Please read below and then fill out the form.

   Once you submit this form you agree to the following terms and conditions.  Please read below  as it has been changed as of 3/13/11.
1.If you drop from the tournament on February 6, 2012 or later you must still pay  entry fee of $300.00
2. If the tournament is canceled due to weather, 1/2 of the entry fee will be refunded.  The other 1/2 will be used towards the next tournament.
3. If 1 day of the tournament is complete then it is considered a full tournament if the other day is canceled due to weather,
4. I know that the entry fee is $300.00 but there is an early payment discount of $50.00.  I also know of the late fee of $50.00. - See the payment info link on the site for more information on the the payment for the tournament.
